📝 Compounding / Extemporaneous Notes

When a commercial liquid is unavailable:
  • Consult a compounding pharmacy for extemporaneous preparations
  • Refer to Allen's Compounded Formulations or PCAB-accredited pharmacy resources for verified formulas
  • USP General Chapter <795> governs nonsterile compounding beyond-use dates (BUDs)
  • Common vehicles: Ora-Plus + Ora-Sweet (1:1), Ora-Blend, Simple Syrup NF, methylcellulose 1%
  • Verify stability data before preparing; many drugs have limited stability in liquid form
Tablet/Capsule crushing — always verify first:
  • Do NOT crush: enteric-coated, extended-release, sublingual, effervescent, or cytotoxic tablets
  • Consult the Non-Crushable Medications reference for a full list
  • Immediate-release tablets that CAN be crushed may often be dissolved in water for administration via NG/G-tube
